Level Up Fitness & Sports Enhancement – Policies, Rules & Facility Waiver

Level Up Fitness & Sports Enhancement has established these Membership Policies, Rules, and Facility Waiver to promote a safe, respectful, effective, and clean training environment for all members, athletes, families, and guests.

By using our facility, booking sessions, enrolling in programs, or signing up for membership, you agree to these policies. It is your responsibility to review and follow the most current version (posted on our website, in-facility signage, and any updates). Level Up reserves the right to update these at any time without notice.

1. Membership & Access Policy

  • A valid membership is REQUIRED to use the facility unless prior permission is granted for a day pass.

  • Day passes: $5 – drop cash in the red box with prior staff approval only.

  • All participants (members, guests, program enrollees) must sign a waiver before using the facility.

  • Memberships provide access as outlined in your plan and auto-renew monthly. 

  • Reservations for turf, courts, cages, or sessions take priority over walk-ins.

  • No sharing memberships.

2. General Facility Rules & Respect the Space

  • Proper athletic attire and non-marking gym shoes are required at all times. No street shoes, boots, or bare feet on turf/court.

  • Re-rack all weights and return equipment after use.

  • Wipe down equipment after each use.

  • No food or drinks allowed on the gym floor/training areas (water bottles only).

  • Clean up after yourself: This includes sweat, spills, personal items, and trash.

  • Respect others: No yelling, foul language, profanity, aggressive behavior, bullying, or harassment. Violations may result in immediate removal and loss of access/membership.

  • No photography or video without express permission from all involved.

  • Report injuries, hazards, or rule violations to staff immediately.

3. Children & Family Policy For liability and insurance purposes

  • No one under 13 allowed in the weight/class area.

  • Children must remain upstairs unless they are actively participating in a supervised class or program.

  • Parents/guardians are fully responsible for their children at all times.

  • Children under 13 must be supervised by a parent/guardian at all times.

  • Ages 13+ may train independently if a guardian has signed the membership agreement and financial guarantee.

  • Youth must have a valid membership or be enrolled in a supervised program to use the facility.

4. Training & Coaching Rules

  • Personal training: Only approved Level Up trainers may provide compensated coaching. Unauthorized training of others may result in membership suspension/termination.

  • Use equipment safely and as intended. No modifications without staff approval.

  • Follow trainer/coach instructions during sessions or programs.

5. Physical Therapy & Rehab Services

  • Services provided in partnership with Pana Community Hospital / Dr. Bryce Fortner.

  • Follow all medical/rehab guidelines.

6. Liability Waiver & Assumption of Risk I understand that sports training, fitness activities, turf/court use, weight training, and physical therapy involve inherent risks of injury, including but not limited to strains, sprains, falls, collisions, cuts, heat-related issues, aggravation of pre-existing conditions, serious physical/emotional injury, paralysis, or death. These risks cannot be fully eliminated.

I voluntarily assume all such risks and release Level Up Fitness & Sports Enhancement LLC, its owners, trainers, partners, staff, and affiliates from any liability, claims, or demands arising from my (or my minor child's) use of the facility, participation in programs, or services—even if caused by negligence (except gross negligence or willful misconduct, as allowed by law).

I agree to pay all medical expenses from any injury and indemnify Level Up against related claims. For minors: Parent/guardian must sign on behalf of the child and agrees to these terms.

7. Enforcement Violations of any policies or rules may result in warnings, immediate removal, suspension, termination of membership/access, denial of entry, or legal action. Level Up may refuse service at its discretion.

8. Party & Facility Rental Policy

Level Up Fitness & Sports Enhancement offers limited private rental of our facility space (including turf, courts, and open areas) for parties, events, team gatherings, birthdays, or other non-training activities on select weekends.

  • Availability: Saturdays and Sundays only, 3:00 PM – 8:00 PM (subject to existing reservations and programs). Rentals are by request only—submit via our online Party Rental Request form.

  • Capacity & Rules:

    • Maximum 20 guests/participants (including adults and children).

    • All renters and participants must agree to these full Policies, Rules & Facility Waiver (linked on our website) prior to booking.

    • Additional rules apply: No food/drinks on turf/court areas (except water); no running in walkway or upstairs, adult supervision required at all times; no alcohol, smoking, or illegal activities; clean up required (additional cleaning fee if needed).

    • Inflatables carry extra inherent risks—follow manufacturer guidelines, supervise closely, and adhere to weight/age limits.

  • Booking & Requirements:

    • Submit request form with date, duration, guest count, and any add-ons.

    • A deposit (amount specified upon confirmation) and signed waiver/agreement are required before access.

    • Level Up reserves the right to deny or cancel rentals for any reason (e.g., scheduling conflicts, safety concerns).

  • Liability: All rentals are subject to the full Liability Waiver & Assumption of Risk in Section 6 above. Renters assume all risks associated with event use, including those from inflatables or group activities.
